Established in 1970, Northwest Human Services has been serving those in Marion and Polk Counties with quality health and social services.  Through seven programs, over 100,000 individuals are served annually.  The mission of Northwest Human Services is to be the leader in providing advocacy, quality health care, and social services to those in need.  We proudly offer comprehensive medical, mental health and social services with compassion and a focus on promoting quality and empowerment with our clients.  We are committed to joining with other agencies to improve community health, public safety and social well-being.  We challenge and foster growth in our staff and maintain financial stability and integrity in our business.

Program Description

 

Connection provides counseling and advocacy services for Deaf, Hard-of-Hearing, and Late Deafened individuals.  Staff helps clients gain emotional health, seek solutions to communication problems, and get help in times of emergency and crisis.

Program Description

 

The Crisis & Information Hotline is staffed 24-hours a day, 365 days a year, providing immediate and confidential intervention for suicide, depression, child abuse and substance abuse.  The information and referral service also connects callers to community agencies that may assist them.  A TTY line is available to serve Deaf and Hard-of-Hearing individuals.  For individuals who are homeless or without a phone, Community Voice Mail provides a voice mailbox for messages from employers, landlords, doctors and others.  Other services available for those in need include limited financial assistance with rent, utilities, gasoline vouchers, bus tickets and life-sustaining medications.  In addition, they also help with diapers and formula, soap and other hygiene items, and after-hours food boxes.

H.O.A.P. (HOMELESS OUTREACH & ADVOCACY PROJECT)

Program Description

 

The goals of H.O.A.P. are to provide case management, advocacy and housing for homeless individuals who have mental illness.  At the Drop-In Center, they provide socialization, showers, laundry, a Monday and Wednesday lunch program and on-site psychiatric evaluations.  H.O.A.P. provides substance abuse treatment, medication management, crisis intervention and supportive counseling.  They also ensure clients receive food, clothing and shelter, and provide advocacy and housing support.  H.O.A.P. is the only mental health service in Marion and Polk County serving homeless individuals who are chronically mentally ill.

 

Safe Haven provides temporary housing for homeless individuals with mental illness.  Staff does outreach to assist those in need to access help.

Program Description

 

HOST Youth & Family Program provides temporary shelter to homeless youth as well as disenfranchised youth in Marion, Polk and Yamhill Counties.  It also provides family mediation and information and referral to youth in crises and their families.

 

The Shelter Program works with youth aged 11-17 and their families.  The primary focus is runaway, homeless and youth/family crises.  Services include temporary shelter for youth, available 24 hours a day, crisis intervention, case management and counseling and mediation with youth and families.  Family life skills classes are given by HOST to facilitate improved relationships between parents and their children.

 

The Transitions Program (TLP) is a long-term 18 to 24-month shelter/housing program for older adolescents ages 16-21.  They receive education as well as life-skill training during their stay as they learn to transition from group to independent living.

 

TOTAL (Transition Options and Treatment Alternatives) provides on-site clinical care coordination and mental health services for youth, ages 12-17, who are transitioning from inpatient care to home environment, or are in need of hospital diversion services.

Program Description

 

West Salem Clinic (WSC) and Total Health Community Clinic (THCC), in Monmouth, are Community Health Centers providing comprehensive medical services to individuals and families of all ages.  Services include health maintenance examinations, well-child visits, childhood immunizations, treatment of acute illnesses and injuries, management of chronic diseases, family planning and annual exams, prenatal care and hospital delivery, in-hospital care from clinic physicians and laboratory services.  Providers and staff deliver medical care in a compassionate and caring manner.
